Application For Financial Assistance
At this time, we are accepting applications from New York, New Jersey, and Connecticut ONLY.
If your pet requires veterinary care and you can’t afford it, please complete this application to determine if you qualify for HEALS financial aid. Please note we don’t cover spay-neuter surgery, routine wellness exams, or annual vaccinations.
To being an application, simply register and sign in and begin an new application.
Along with the application send the following:
- Last 2 Pay Check Stubs or Proof of Disability and or Social Security*
- Lastest Tax Return First and Last Page*
- Care Credit Acceptance/Denial
- Scratch Pay Acceptance/Denial

The cost of veterinary care has risen tremendously. While we help our qualifying clients to the extent possible, in most cases we aren’t able to cover the cost of the entire veterinary bill.
